Collection and Shipment of your Specimen

Read all Instructions before obtaining sample

1. Fill out the enclosed Registration form. The information on this form is necessary for accurate evaluation.



2. As soon as you receive the kit, remove the blue and red tag temperature recorder that is taped to the white cardboard box and put it in a safe place for later use in step 8. Put the ice cube pack in the freezer. Put the entire Styrofoam box containing the cool packs and preservative solution into the freezer. Do not freeze the ice brick separately since it will reform into a different shape and then not fit inside the Styrofoam box. When you are ready to use the kit, take out the small bottle containing preservative solution and place it in warm water for 10 minutes, allowing it to thaw and warm up. Your specimen must be shipped with Airborne Express, using the Air Bill provided. This has already been paid for by us. Airborne Express works closely with us to ensure the most reliable service.



3. Call 1-800-AIRBORNE to arrange for a pick-up at your location, Monday through Thursday only. Shipments are not to be made on Fridays, Saturdays or Sundays.



4. Your Semen Sample must be shipped the day it is collected. It is recommended that you produce your sample within 2 –3 hours before the pick-up. If possible abstain from ejaculating for 2-3 days (no more or less) before providing your semen test sample; this will ensure the highest possible sperm count.



5. Do not use Spermicidal Lubricants, Saliva or any products containing Spermicides i.e. Condoms.



6. Masturbate and ejaculate directly into the the larger plastic bottle labeled A (specimen). Try to collect the whole ejaculate into the bottle. If any part is missed, report in on the registration form. This will ensure proper evaluation of your sample. Open bottle B, after it has thawed and warmed in warm water. Then peel off the metal cap carefully so you do not cut your finger. A Add the contents of bottle B (preservative solution) to bottle A, which now contains your semen sample. Gently swirl for 10 seconds to ensure adequate mixing. Discard empty bottle B in your household garbage.



7. Firmly twist the cap onto bottle A to avoid leakage during shipment. Place bottle A into the small sandwich sized biohazard bag.. Seal the biohazard bag and place it into the styrofoam box next to the ice.



8. Pull the tab on blue and red tag that you saved from step one and place it between bottle A and the grey foam inside the styrofoam container. This is a temperature recorder which monitors that the semen sample did not get to warm or too cold.



9. Put the Styrofoam lid back on top and place the entire Styrofoam box back into the white cardboard box. Place it into the Airborne Express Lab Pack (green bag). Our address will be pre-printed on the Airborne Express Airbill. Call Airborne for your pick-up.
